The Contact Us Portlet is a data collection tool. The portlet enable you to create a form that collects information from website visitors. (Users must fill-in and submit the data in order for it to be collected). The form fields are customizable allowing you to collect information that is relevant to your viewing audience.

1- | Contact Name: | Title of the portlet being created.This is also the title of the Contact Us portlet that is viewed on the web page. |
2- | Short Description: | Provide a short explanation of the portlet you are creating. |
3- | Display Profile Information: | Setting this option to 'Yes' causes the rendered Contact Us Portlet to display a user's profile information (first/last name, email, address) if that information is in the session for a given user. |
4- | Email Address: | Enter the individual'a email address who will receive this form when sent. |
5- | Contact Response: | A message the client will receive after submitting this form. |
6- | Contact Header: | A line of text about the Contact Us form that indicates what the information below is about. |
7- | Collection Statement: | Content explaining to the individual submitting the form that they must agree to a privacy policy. A check box appears next to the statement and they are required to check the box before submitting the form. |
8- | Opt-In/Consent Statement: | Content giving permission to the company that they can be contacted. There is a check box that appears next to this statement and its required to be checked before the form is sent. For example: I understand that "Company Name" may contact me with more information about its products and services. |
1- | Profile Data Field Name: | Fields you chose that the client has the option to fill out when |
2- | Order: | Order the fields the way they should appear on the form. |
3- | Required: | Determine whether or not the field is required for the customer to fill out. A form cannot be submitted until all required fields are filled out. |
Add a Contact Us Portlet

Select Portlets: Contact Us.

Select + adjacent to Portlet Actions for Contact. The Contact Portlet screen displays.

Identify the contact individual and provide a short description of the form you want to build.

Select the drop-down arrow to have Web Crescendo display their contact information on the form.

Set the Contact Response. Select the + adjacent to Contact Response, then in the white stage, enter the text you want to display.

Enter the Header of the Contact Form. From the Contract Response window, scroll to the Contact Header selecting the +. Enter the Header as you would like it to display on the form.

If users must agree to a Privacy Policy(About the two types of privacy you can build), select the + adjacent to Collection Statement, and enter the policy in the white staging area.

If users must Opt-In to receive any marketing campaigns, click the +adjacent to Opt-In Consent Statement. You may need to scroll down to view the Opt-In Consent window where you enter the text you want users to view.

Click Save Info.
